    What You Will Do as a Hospice Nurse Practitioner

    As a Hospice Nurse Practitioner, you will collaborate with the Hospice Medical Director and provide services related to health promotion, chronic disease management, and end of life care.


    Some of your primary responsibilities will include:

    • Performs face-to-face encounters with the patient to determine continued eligibility for hospice care within time frame of regulatory guidelines.
    • Documents clinical findings in detail at patient bedside as related to the hospice criteria and provides a clinical report to the hospice physician within 24 hours of visit.
    Prepares and maintains accurate patient interdisciplinary team and services provided.

    • Complies with legal, regulatory and accreditation requirements.
    • Evaluates hospice patients for appropriate level of care.
    • Assists in development of hospice protocols, policies and procedures as required.
    • Participates in QAPI activities as requested.
    • Participates in in-service education related to areas of practice as required.
    • Addresses and supports cultural practices, as long as practices do not harm others.
    • Adheres to all organization, payroll, billing and documentation policies and procedures.
    • Knowledgeable of current federal, state and accreditation standards and organization regulations concerning patient care.
    • Adheres to MSA policies and procedures.
    • Completes annual educational requirements.
    • Ensures compliance with state and federal regulations, company policies/procedures and process maps in the performance of job responsibilities.
    • May provide on call support to staff.
    • Contributes to team effort by accomplishing related results as needed.

    Education, Licensure & Certification Requirements


    • Previous hospice and palliative care experience preferred.
    • Excellent oral and written communication skills.
    • Master's degree in nursing.
