    POSITION SUMMARY:

    Sign-on bonus starting at $6,000

    Plans, evaluate and administers physical therapy services and care to patients per physician's orders. Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as, Health System initiatives. Coordinates family involvement in assessment and treatment of the patient.

    EDUCATION/TRAINING:

    Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation). Participates in educational programs on an ongoing basis and complete mandatory CEUs as required.

    LICENSURE/CERTIFICATION:

    Current license from the State of Illinois or in the process of acquiring licensure. An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. CPR certification required.

    EXPERIENCE/SKILLS/ABILITIES:

    Successful completion of annual clinical, age and job specific competencies and skill verification tools required. Ability to effectively communicate orally and in writing in a caring, considerate and concerned manner. Willingness to accept responsibility. Ability to direct ancillary staff and organize work assignments. Leadership skills essential to the position. Must be skilled in a broad range of modalities with a clear understanding of indications and contraindicating of each. Familiar with the indications and use of equipment. Guides, teaches, and supervises students.

    ESSENTIAL FUNCTIONS:
    1. Evaluates patients and establishes a physical therapy plan of care that integrates goals, treatments, outcomes and discharge plans.
    2. Administer plan of care incorporating age-specific, cultural and spiritual needs of the patient and significant other.
    3. Continually observes and reevaluates treatment effects consulting with physician and other health care professionals, altering program as needed in order to provide quality patient care.
    4. Supervises and delegates assistants, technicians, students and volunteers in providing support functions.
    5. Coordinates appropriate patient referrals for other services as needed and functions as an advocate for appropriate services and resources during stay and in preparation for discharge.
    6. Educates patients, family and other health care providers on appropriate care modifying activities considering learner's needs and capabilities and conducts ongoing educational programs as required.
    7. Accurately documents patient information and treatment following department policies and maintaining confidentiality of position acquired information. Completes billing in a timely manner.
    8. Performs all aspects of care in an environment that optimizes patient and environmental safety and in accordance with applicable laws, regulations and compliance efforts.
