    Summary



    The Licensed Practical Nurse provides specialized, skilled and individualized practical nursing care to all levels of patients including most complex patients who are acutely ill and/or chronically ill.

    Care is provided to patients who exhibit a variety of most complex physical and/or behavioral problems. This position will service the Wilmington VA Medical Center.

    Duties




    DUTIES:


    The LPN, GS-6 in Primary Care (PC) provides skilled and individualized practical nursing care to all levels of patients including most complex patients who are acutely and/or chronically ill in the outpatient care setting.

    The Licensed Practical Nurse (LPN) clinical associate position is located within a Patient Aligned Care Team in Primary Care Service, at Wilmington VAMC.

    Care is provided to patients who exhibit a variety of most complex physical and/or behavioral problem.

    This person functions under the direct supervision of the Team RN who is responsible for the coordination of the patient's care.

    The LPN GS-6 has the knowledge and skills necessary for self-direction in carrying out simple to the most complex practical nursing care.

    The incumbent serves as a member of a health care team/ team let in Primary Care. The LPN recognizes the need for and seeks guidance when necessary to ensure quality patient care. In organizing and delivering care, the LPN recognizes and considers emotional, cultural, spiritual, and age related factors.

    The LPN actively seeks out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.

    The incumbent is accountable for all patient care given and functions within the LPN scope of practice as it relates to Primary Care polices.

    The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic life care plan.

    The LPN demonstrates active collaboration and communication with the Core Team (Patient, Primary Care Provider, RN Care Manager and Clerical Associate) and Expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient)

    Monday - Friday 8:00 am - 4:30 pm
