    Job Description


    • As a Hospice RN/RN Case Manager, you will be responsible for providing direct patient care services to hospice patients and supportive care to their families.
    • You will act as the primary liaison between the physician, patient/family, and hospice team, ensuring seamless communication and coordination of care.
    • You will empower patient caregivers by educating them on how to provide care for their loved ones, promoting independence and confidence.
    • Manage and collaborate with an interdisciplinary team to provide comfort, peace, and dignity to patients and their families.
    • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
    • Maintain all assigned patient clinical records and participate in company quality assessment and performance improvement programs.

    About You


    To succeed in this role, you should have:

    • A compassionate heart and empathetic nature, with a strong commitment to delivering high-quality patient care.
    • Flexibility and composure in a fast-paced environment, with the ability to support others in times of crisis.
    • An active RN license in the state where you are employed (or eligibility to obtain).
    • At least one year of nursing experience in hospice, home health, ICU, geriatric, or oncology, with additional experience preferred.
    • Current driver's license and automobile insurance.
    • Current CPR certification.
