    Job Description:

    We require an Occupational Therapist to develop, recover, or maintain daily living and work skills of individuals with physical, mental, or developmental conditions. As a key member of our team, you will provide client-centered services enabling patients to confidently perform everyday tasks or activities.

    Responsibilities:

    • Assess care plans and create OT Home Care treatment plans alongside physicians or by prescription
    • Document visits and updates on patient care and progress
    • Educate caregivers and their families on patient care
    • Making informed decisions on patient care and services needed
    • Support the company mission by delivering exceptional home care services

    Qualifications:

    • OT License and Registration to practice in New York State
    • Bilingual skills highly valued
    • At least 1-year experience as a Occupational therapist in home care, preferably with Medicare Part A experience
    • Reliable transportation
    • Excellent communication, organizational, and time management skills
    • Problem-solving skills and ability to work independently
    • Proficiency in using HomeCare HomeBase (HCHB) software
