    At Liberty Home Care, we strive to provide patients with the best possible care in the comfort of their own homes. Our Physical Therapy Assistants play a vital role in helping patients recover from illnesses, injuries, or surgeries.

    Key Responsibilities:

    • Provide patient care services and direct treatment as assigned by the Physical Therapist.
    • Update care plans in coordination with the therapist and physician to reflect progress and adjustments.
    • Plan monthly patient re-evaluations with the therapist and coordinate weekly schedules as needed.
    • Document patient care data accurately and timely, reflecting improvements or changes in condition.
    • Submit daily/weekly visit schedules to the Scheduler and communicate with intercompany contacts.
    • Discuss treatment progress with the therapist, patient/family, and physician as necessary.
    • Coordinate referrals to other disciplines and attend mandatory in-services and meetings.

    Requirements:

    • Completion of an accredited physical therapy assistant program and current State licensure.
    • Minimum one year's therapeutic experience in a non-home health care environment; two years preferred.
    • Strong knowledge of physical therapy theory and concepts, with ability to treat patients with physician orders.
    • CPR certification and excellent communication and organization skills.
    • Twelve hours of continuing education annually.
    • Acceptance of travel and working conditions as assigned.
