    Family Nurse Practitioner (NP) or Physician Assistant (PA) - Outpatient

We are currently seeking a dedicated and compassionate Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient team.     As a Family Medicine Nurse Practitioner or Physician Assistant in our outpatient department, you will play a pivotal role in delivering comprehensive healthcare services to our patients. You will diagnose and treat a wide range of medical conditions, provide preventive care, and promote overall health and well-being.     Key Responsibilities:

    1. Patient Care:
      • Conduct thorough medical evaluations, examinations, and assessments for outpatient clients.
      • Diagnose and treat a variety of acute and chronic illnesses.
      • Develop and implement individualized treatment plans.
      • Provide preventive care, health screenings, and vaccinations.
      • Manage and monitor chronic conditions, including diabetes, hypertension, and more.
      • Offer counseling on lifestyle modifications, nutrition, and exercise.
    2. Medical Records and Documentation:
      • Maintain accurate and up-to-date electronic medical records.
      • Document patient encounters, diagnoses, treatments, and progress notes in accordance with medical standards and regulations.
    3. Collaboration and Consultation:
      • Collaborate with other healthcare professionals, including physicians, nurses, specialists, and support staff, to ensure coordinated patient care.
      • Consult with specialists as needed and facilitate referrals for further evaluation and treatment.
    4. Patient Education:
      • Educate patients and their families about their medical conditions, treatment options, and preventive measures.
      • Encourage and empower patients to actively participate in their healthcare decisions.
    5. Quality Assurance:
      • Adhere to evidence-based guidelines and best practices to deliver high-quality care.
      • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
    6. Continuing Medical Education:
      • Stay current with medical advancements, research, and industry trends.
      • Engage in ongoing professional development and educational activities.

    Qualifications:

    • Master's degree in Nursing (NP) or Physician Assistant (PA) studies from an accredited program.
    • State licensure as an NP or PA with a focus on Family or Internal Medicine.
    • Strong clinical skills with a commitment to evidence-based practice.
    • Excellent communication and interpersonal skills.
    • Empathy, compassion, and a patient-centered approach.
    • Ability to work effectively and collaboratively in an outpatient healthcare team.
    • Commitment to maintaining patient confidentiality and upholding ethical standards.
    • Prior experience in an outpatient or primary care setting is preferred.
