Job Description
Job DescriptionSummary of Position: The Community Outreach Specialist is responsible for building relationships with key contacts at the assisted living facilities, nursing home facilities, hospitals, and physician offices for adding clients to State of the Heart Care programs. The Community Outreach Specialist is responsible for educating the community and referral sources about State of the Heart services.
Education and Experience:
1. High school diploma or associate degree in business or a related field.
2. Minimum of two years of Hospice or community health experience.
3. Healthcare sales and/or marketing experience preferred.
Special Skills and Requirements:
1. Shall have a valid driver's license.
2. Excellent customer service skills.
3. Detail oriented with excellent communication, documentation, and organizational skills.
4. Ability to follow instructions.
5. Ability to work independently without supervision.
6. Travel to other sites as needed. Travel may be in inclement weather.
7. Ability to lift 25 lbs of supplies or equipment.
8. Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking.
