Clinical Generalist Pharmacist; DH – Fort Wayne, IN

Our client, a large healthcare entity in Northwest Indiana, has a need for a Clinical Generalist Pharmacist in Fort Wayne, IN.

The clinical generalist will provide pharmaceutical care services to the patients of 2 hospitals locations in Central and North Fort Wayne.

Clinical Generalist rotate through clinical and distribution services, however, anticipate being assigned to a clinical pharmacist role >60% of the month.

Do you possess a pharmacy degree from an ACPE-accredited institution?
Have you completed a PGY1 pharmacy residency or have equivalent hospital experience?
Are you licensed in Indiana or eligible to transfer a current license?
Can you reliably commute to Central and North Indiana on a full-time, on-site basis?

Main Responsibilities:

Multidisciplinary rounds 7 days per week.
Oversight, monitoring, preparation, dispensing, and proper documentation of patietn medication therapy
Advising physicians on matters pertaining to drug usage and control.
Providing drug informaiton, patient information, and oversight of information and dispensing processes and systems is also critical.

Clinical practices include:
dosing and monitoring anticoagulants, antimmicrobials including vancomycin, total parenteral nutrition, patient-controlled analgesia, and others.

Generalist have ample opportunity to mentor pharmacy IPPE/APPE students, 5 PGY1 pharmacy residents and 1 PGY2 ambulatory care pharmacy resident.

Education Requirements:

Candidates must possess a pharmacy degree from an ACPE-accredited institution
Candidates must have completed a PGY1 pharmacy residency or have equivalent hospital practice experience
Pharmacy licensure, or eligibility, in Indiana is required
