- Setting: Transitional Living Residence
- Schedule: Monday to Friday
- Hours: 8:30am to 4:30pm or 8:30am to 5pm
- Patient Population: Caseload of 5 to 6
- Coordinate the discharge of residents, tracking progress toward discharge
- Provide ongoing individual and group meetings, family and other collateral contacts, as needed
- Coordinate residents' benefits, TSOCR rent, PNA, state Medicaid, etc.
- Track / apply for residents birth certificates, social security cards, and resident alien cards
- Prepare required documentation including but not limited to Service Plans, Service Plan Reviews, Admission Notes, Discharge Summaries, MHARS 725s/ 116s, and monthly Progress Notes
- Arrange and provide tours of Transitional Living Residence
- Attends the Social Work Department meetings, bi-monthly teleconferences with discharge conferences, case conferences and individual & group supervision
- Escort residents to housing interviews / tours and admissions to new residences, as needed
- Coordinate and follow up on SSI/D benefits as well as State and community Medicaid applications
- Perform other duties, as needed
- Bachelor's and/or Master's Degree in Social Work
- Prior experience working in a Mental Health and/or Social Services setting
- Experience with Case management
- Experience working with Adult Mentally Ill population
- Solid assessment and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
