    Summary



    The primary purpose of the Peritoneal/Hemodialysis position is to demonstrate leadership, clinical competence through providing direct care and case management to people with chronic kidney disease, provision of treatments, continuous CRRT, patient and family education, assessment, and management of peritoneal and/or hemodialysis catheter access, and provision of the holistic needs of the patient under his/her charge.



    Duties


    PRACTICE: Uses the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation). Accurately documents care of clients.

    1. Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care (ANA Code of Ethics for Nurses).

    2. Delivers safe care to assigned patients, while developing technical competencies.

    • a. Safely and effectively uses patient care equipment.
    • b. Correctly and safely performs nursing procedures.
    • c. Recognizes overt abnormal signs and symptoms and/or changes in patient's condition and responds appropriately.
    • d. Implements provider's orders in a timely and complete manner.
    • e. Implements nursing interventions/orders in a timely and complete manner.
    • f. Requests assistance from co-workers when need; knows where to ask for help.

    3. Formulates a plan of care based on nursing assessments, using the nursing process, based on the ANA Standards of Nursing Practice.

    • a. Completes initial assessment of patient condition and responses to treatment and initiates follow-up as needed.
    • b. Includes the patient and family/significant other in developing the initial plan of care and establishing goals.
    • c. Updates plan of care with assistance as needed.
    • d. Assesses patient's learning needs, learning preference and readiness to learn.
    • e. Develops and implements a plan to meet learning needs, instructing patients and families in basic health teaching and discharge planning.

    4. Documents the nursing process.

    • a. Documentation is consistent, accurate, concise, and timely.
    • b. Evaluates and documents patient response to interventions and progress towards attainment of goals and expected outcomes.
    • c. Revises plan of care according to evaluations of patient's responses and outcomes.
    • d. Uses appropriate software per policy, as well as any future patient documentation programs.

    5. Demonstrates grade-appropriate and consistent critical thinking skills.

    • a. The nurse can recognize the patient has a problem.
    • b. The nurse can manage the problem safely and effectively, recognizing his/her scope of practice.
    • c. The nurse as a relative sense of urgency.
    • d. The nurse takes the right action for the right reason (del Bueno).

    6. Position-specific Competency Checklist is completed annually and is rated competent in all measured areas.
