    LVN, Licensed Vocational Nurse - Primary Family Medicine

    You are responsible for providing triage (sorting/prioritizing patients) over the telephone to assist in assessing patient care with physician.

    Responsibilities:

    • Takes and records vital signs, weights, wound measurements, etc. Identifies trends outside of parameters, or in need of attention and communicates to RN/physician
    • Reports pertinent information and changes in individual health status to attending physician/health professionals and to the Team as needed in a timely manner
    • Ensures the provision of needed adaptive equipment
    • Administers medications and treatments as directed. Responds to emergencies and provides care.
    • Directs Certified Nursing Assistants in performance of work.
    • Functions as shift Charge Nurse as assigned.
    • Responsible for providing timely referral determination by accurate:
    • Usage of the Milliman Care Guidelines
