- Provide case management for an assigned group of members including coordinating and monitoring the activities of the Individual Treatment Team (ITT)
- Develop, implement, evaluate and revise overall treatment goals and plans in collaboration with the member and ITT
- Provide individual supportive therapy and symptom management
- Ensure immediate changes are made in the treatment plans as members change
- Educate and support member families and advocate for member rights and preferences
- Participate in and conduct parts of the client-centered comprehensive assessment of psychiatric history (i.e. onset, cause and effect of illness, past treatment and responses and risk behaviors, mental status, and other Social Determinants of Health)
- Associates degree in Nursing

As our Psychiatric RN on our ACT multidisciplinary team, you will be responsible for conducting psychiatric nursing assessments, assessing physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the psychiatrist, and sharing shift-management responsibility with other staff.
There is very little office-based work in this role, so the perfect candidate will be comfortable performing traditional nursing duties (blood/lab draws, medication management/LAI application, psychoeducation) in the community with their clients. This team covers Wake County - we provide mileage reimbursement
Pay
Starting pay is $36+ per hour, based on experience.
$2,000 sign-on bonus

Schedule
Flexible, 40 hours per week.
Daily morning meetings at 8:30am - create your own schedule otherwise

You will be eligible for medical benefits the first of the month following your first 30 days. We have options for a standard plan, as well as an HSA with a $300 initial contribution and up to a $25 per paycheck match. You would receive 10 paid holidays as well as PTO hours which would accrue at a rate of 10 hours per month. After 6 months you would be eligible to enroll in our 401(k) plan which has a 4% match. We also provide company paid life insurance, short- and long-term disability, in house and paid training opportunities, and paid credentialing. SouthLight is also an approved employer for the Public Service Loan Forgiveness Program (PSLF).
