    Description

    • Provides psychosocial assessments and treatment to patients and or families related to adjustment to illness and discharge planning.
    • Provides education and support to patents and their families to encourage compliance with medical and behavioral regimes.
    • Provides individual, family and group therapy as appropriate to setting.
    • Assists with assessment and treatment of behavioral issues in the hospital, including consultation.
    • Maintains effective communications with staff and attending physicians related to patient care, psychosocial and psychiatric needs.
    • Provides brief interventions utilizing evidence-based techniques
    • Provides behavioral activation, motivational interviewing and cognitive behavioral techniques.
    • Documentation consistent with established policy and procedures to create a permanent patient record.
    • Assists, and serves as a resource to the case management team with referring to treatment or community agencies as appropriate.
    • Provides group therapy to inpatients daily as required by schedule
    • Provide case management, referring to treatment or community agencies as appropriate.
    • Maintains and provides current information and referral services to patients, caretakers and families
    • Provides appropriate community resources and agencies and units as appropriate.
    • Provides education to patients, interns, medical students, Nurse Practitioners, Physician Assistants and Social Workers.
    • Collaborates with Consult Liaison team to facilitate the management of complicated mental health cases.
    • Consultation with team members in the hospital on the assessment, treatment and discharge planning of patients problems.
    • Participate in scheduled rounding of patients identified for referral to CL services.

    • Informs RNs, Leadership, Psychiatrist of pertinent clinical information obtained via individual/group therapies or assessments.

    Work in a diverse environment that offers individuals the ability to interact with clients/patients in both mental and addiction recovery environment.

    Work as a member of a multidisciplinary team in providing psychiatric care as a consultant to a large medical/surgical inpatient service.

    Work collaboratively with hospital staff, providing liaison activities such as educational offerings, assistance with behavioral management of patients, and debriefings.

    Assists discharge resource team in coordinating transfers to outside psychiatric hospitals as needed and aftercare planning for psychiatric needs. Provides short term therapy for patients with mental health and/or addiction disorders.
