    The Physical Therapist (PT, DPT, RPT) in Home Health will be responsible for assessing and evaluating patient care needs concerning functional status, mobility, endurance, pain, and other medical conditions. Based on the assessment, the Physical Therapist will collaborate on developing a treatment plan, implementing interventions to enhance the patient's well-being, and monitoring progress.

    • Evaluate the patient's functional status and therapy requirements. Collaborate with the physician on the treatment plan.
    • Observe and communicate the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
    • Provide treatment to alleviate pain, improve and restore function, or maintain maximum functionality following best practice guidelines.
    • Educate and update team members, patients, and family/caregivers on the care plan, limitations, and progress towards objectives.

    Licensure Requirements

    • Active Physical Therapy license in the state
    • Valid CPR certification
    • Current driver's license, vehicle insurance, and reliable vehicle access
