    Description

    The functions and responsibilities are to work with various departments within HMHC and outside agencies to divert consumers from inpatient or residential placement if possible.

    If a consumer is admitted to inpatient or residential services the responsibilities of the position include coordinating discharge and follow up from inpatient or residential treatment in order to decrease the likelihood of readmission.

    Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.


    ESSENTIAL FUNCTIONS:

    • Be aware of which consumers of HMHC services are at risk for inpatient or residential placement.
    • Work with departments of HMHC towards diverting consumers from inpatient/residential treatment if at all possible.
    • Coordinating services within HMHC and within our service area communities in the effort to divert a hospitalization or residential placement or to coordinate services within HMHC and the consumer's community in order to decrease the likelihood of readmission.
    • Follow consumer through diversion and/or follow up after release from hospitalization until the consumer is stabilized and the risk for admission or readmission has diminished.
    • Maintain communication within HMHC and with community agencies regarding the at risk consumer - in person, via phone, and email.
    • Participate in weekly/biweekly/quarterly meetings with hospitals and PRTF's to receive updates on consumers progress and discharge planning.
    • Complete yearly Continued Stay Screens.
    • Provide preadmission information to consumers scheduled to receive services.
    • Relates with: Works closely with all staff of HMHC, especially staff providing direct services to consumers. Works particularly close with direct service staff in screening services and stabilization services.

    MINIMUM EDUCATION, KNOWLEDGE AND SKILLS REQUIRED:

    • Good organization skills
    • Proficient in Microsoft Office (Outlook, Word, Excel, PowerPoint, etc.)
    • Education: Bachelor's degree in mental health related field or 4 years of experience in related field. Working knowledge of public mental health services in Kansas preferred.
    • State required Targeted Case Management and Case Management trainings.
