    ACTT is a comprehensive team of professionals from many disciplines that provide tailored services to meet the client's mental health, substance use, vocational and housing needs.

    ACTT improves outcomes for people with severe mental illness who are most at risk of psychiatric crisis and/or affected by other social determinants of health.

    Providing Care to Those Who Need it Most.

    ACTT provides treatment and care to patients who may not have access to these important services otherwise. The patients they help have some of the highest risks for hospitalization, homelessness, or incarceration.
    We are seeking an RN to Join or ACTT Team

    WORKS WITHIN A TEAM TO PROVIDE:

    • TREATMENT TEAM
    A. Participate in regularly scheduled treatment team meetings
    B. Provide clinical expertise and interact with multi-disciplinary treatment team
    C. Attend daily staffings and update team members with relevant information


    • CONTACT WITH REFERRAL SOURCES
    A. Maintain regular contact with referral sources as indicated
    B. Update on consumer's progress
    C. Develop appropriate discharge plans collaborating with referral sources


    • MEDICAL\MEDICATION MANAGEMENT
    A. Coordinates consumer needs with health care providers
    B. Monitors Medication compliance, adverse effects
    C. Gives IM injections per prescription to consumers
    D. Coordinates pharmaceutical inventories, supplies.
    E. Coordinates with registered pharmacists to meet agency needs
    F. Maintain doctor schedule to ensure clients are seen regularly
    G. Perform vitals on clients seeing the doctor and complete AIMS at admission and every 6 months
    H. Complete PAP on clients as necessary
    I. Prepare pillboxes daily, weekly, bi-weekly, monthly as appropriate
    J. Assist psychiatrists as needed while they are seeing clients. Work closely with doctor to assure medication refills are up date
    K. Provide medication education to staff/ clients as needed


    • CASE MANAGEMENT
    A. Provide transportation for clients to access community resources
    B. Emergency services/ on call duty on rotation that may include commitment procedures, after hour assessments, crisis planning, and hospital diversion
    C. Travel to community to see clients and provide needed assistance


    • DOCUMENTATION
    A. Responsible for timely completion of medical record documentation
    B. Documentation of all outside contacts as well as therapy sessions will be maintained
    C. Update green sheets in the medical chart each time the client is seen by the doctor
    D. Update green sheets in crisis notebook each time there is a change

    A. Attend and participate in one or more CQI teams annually
    B. Participate in peer record review process

    Profile:
    (Knowledge/Professional/Technical

    Competencies)
    Prefer Bachelor or Graduate degree in nursing

    Experience:
    Must have at least 2 years of experience working with individuals with severe and persistent mental illness.

    Prefer QP status according to 10A NCAC 27G.0104 or be an Advanced Practice Nurse (APN) according to NCGS Chapter 90 Article I, Subchapter 32M.
