About the Role:
We are seeking a well-trained and compassionate Primary Care Physician (MD/DO) to join our growing care team.This position provides a unique opportunity to practice within a multidisciplinary care model designed to proactively manage primary care needs along with Chronic Disease Management.
This position is a clinic-based setting with the ability to provide both consultative and longitudinal care, supported by care coordination teams and advanced technologies.
- Deliver comprehensive primary care services
- Conduct patient assessments through medical history, physical exams, and diagnostic evaluation
