- Develop resident care plans based on the comprehensive MDS assessment.
- Ensure that all regulations are met regarding the MDS assessment process and obtains appropriate signature for all assessments.
- Coordinate discharge dates and participates in the discharge planning process.
- Maintain MDS computer system and equipment.
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Licensed Practical/Vocational Nurse obtained prior to hire date or job transfer date required.
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
- Long-term care experience preferred.
