JOB SUMMARY:
The Clinical Pharmacy Technician will perform delegated pharmacy tasks to ensure the high functioning of clinical pharmacy programs under the general supervision of a Clinical Pharmacy Specialist.
The Clinical Pharmacy Technician will conduct outreach to assigned patients to collect information requested and schedule medication therapy management appointments with the Clinical Pharmacy Specialist(s).
The Clinical Pharmacy Technician will work collaboratively with a multidisciplinary care team which includes nurses, social workers, and medical providers to assist with coordination of care.
MAJOR DUTIES AND RESPONSIBILITIES:
Patient Focus
Provide direct outreach to patients, providers, pharmacies to collect accurate, pertinent information and update systems as appropriate
Schedule patients for appointments with the Clinical Pharmacy Specialist(s)
Provide follow-up outreach, including telephone and/or letters for appointment reminders
Coordinate medication refills following directions from Clinical Pharmacy Specialist/Medical Provider
Handle and resolve any concerns related to medical insurance
Checking, Examining, and Recording
Works collaboratively with the pharmacy team to gather and document data
Review and manipulate data in various platforms such as Excel, electronic medical records, and population health platforms
Maintain copies of reports for quality improvement and compliance purposes
Analytical and Critical Thinking
Develop presentations, charts, and workflows in collaboration with the pharmacist
Compliance, Policy, and Procedure
Review and understand pharmacy policy and procedures and regularly seeks to adhere to them
Professionalism
Treats patients and colleagues in a respectful and confidential manner
Provide professional and excellent customer service
Performs other related duties as assigned.

EDUCATION AND/OR EXPERIENCE:
Relevant Bachelor's degree, or equivalent combination of education and experience is required.
Current Maryland state pharmacy technician registration in good standing and Certified Pharmacy Technician (CPhT) recognized by the Pharmacy Technician Certification Board (PTCB) is required.
PTCB Medication History certification required or willingness to pursue within 90 days of hire.
Current CPR/BLS certification
2+ years of experience working in a clinical practice setting is preferred.
