- Oversee the coordination and plan of residents' care, ensuring practices prioritize each resident's best interests
- Complete clinical assessments of residents' health status and document accurately
- Monitor chronic illnesses and oversee medication administration
- Collaborate with external health care providers and internal team members
- Communicate with residents' families, reporting any changes in health or care plans
- Licensed as a Registered Nurse / RN in Minnesota
- Proficient in Electronic Medical Records (EMR) software
- Previous nursing experience in a SNF, AL, TCU, or Home Health setting preferred
- Ability to work independently
- Strong skills in planning, organization, and communication
