Job summary
The Foster Care Case Planner works toward the goal of permanency for children either through reunification with parents and if not possible through adoption or preparation for independent living.Responsibilities
- Engage with birth parents foster parents and foster youth using motivational interviewing techniques and assess for safety and risk factors for children currently in care and in planning for permanency.
- Implement evidence-based models with foster parents birth parents/discharge resources and youth This includes participating in family conferences supervising family visits conducting drug treatment medical school housing referrals accompanying families to settings working closely with socio-therapist advising counseling advocating case planning developing safety plans utilizing crisis intervention skills maintaining caseload attending court proceedings coordinating facilitating family sibling visitations/meetings providing outreach/advocacy services engaging absent hard-to-reach family members acting as resource referrals community services typing computer literacy required excellent interpersonal skills valid driver license BSW MSW child welfare experience preferred knowledge community resources negotiation systems multi-problem families drug treatment populations bilingual Spanish English preferred Education bachelor degree U S accredited educational institution credential evaluation non-U.S. educational institution hiring impact changing lives together
