- Work in various settings such as Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree or equivalent experience and NP licensure
- Current RN and NP licensure with prescription authority
- Board certified in a Matrix approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and communication abilities
- Ability to travel and drive a car

About the Position:
During comprehensive health assessments, Matrix providers review a member's health, medical history, environment, and risks. This gives valuable insight that may be missed in routine office visits, helping to improve care quality and close care gaps.

Nurse Practitioner - Part Time - Home Risk Assessments
Type: Part Time - Flexible-Nurse Practitioner
PAY: $90.00 or $95.00 (based on county) per Assessment
Location: Avon Lake, Ohio
Hours: Days, Evening, Nights, weekend, Flexible Scheduling
Benefits: 401(k) and limited Voluntary Benefits for PRN/Part Time providers
Mileage Reimbursement plan
