Heritage Health seeks to provide access to its community by offering expanded hours for acute care needs, including weekends, to reduce utilization of urgent care and emergency rooms. As a Provider working in a walk-in/Rapid Care clinic environment, you will provide comprehensive, patient-centered care to individuals and families with acute and chronic needs requiring immediate access to care. This role will not carry a patient panel. You will deliver high-quality medical services, including health assessments, diagnosis, and treatment of acute and chronic conditions, preventive care, and patient education for our diverse patient population, including pediatrics. By ensuring accessible, equitable, and culturally responsive care, you will support our mission to reduce health disparities and improve the overall health and well-being of the communities we serve.

Salary:

NP/PA $129,422.00

Requirements

Minimum qualifications:

• Graduate of an accredited Nurse Practitioner (FNP, ARNP) or Physician Assistant program
• National certification as NP, PA (NCCPA)
• Current, unrestricted license to practice in the state of Idaho (or ability to obtain prior to start). No litigation or disciplinary actions may be pending against the incumbent.
• Current DEA Registration (or ability to obtain prior to start)
• Current Basic Life Support (BLS) Certification
• Current Controlled Substance License (or ability to obtain prior to start)
• 2 or more years of experience as a primary care provider is preferred. Urgent care experience is preferred but not required.
• Willingness to work weekends as part of a 3 (three) 12-hour patient-facing shift, or part of a 4 (four) 10-hour shift.

Knowledge, Skills, and Abilities Required:

Your Essential Duties:

• In accordance with standard of care and within scope of practice, provides care to patients through diagnosis, treatment, and management of acute and chronic problems in accordance with standards of care as described in Principles of Practice, protocols, and scope of practice as approved by supervising physician and Board of Medicine.
• Consult with other clinic providers, specialists, and health care professionals as appropriate.
• Directs and guides support staff in delivering coordinated patient care.
• Collaborates with other health care providers as part of a multidisciplinary team to provide comprehensive patient care.
• In coordination with other clinic staff, maintains system of quality assessment and assurance in accordance with quality assurance plan.
• Participates in program planning, development, implementation and evaluation relating to specific job, clinic, and program functions that occur during regular business hours regardless of scheduled, patient-facing days in clinic.
• Participates in staff meetings, conferences, and in-service training and continuing education.
• May be asked to participate in a shared call system with other providers that occur during regular business hours regardless of scheduled, patient-facing days in clinic.
• Responsible for accurate, timely, and complete documentation in patient records. Including providing appropriate code level and diagnosis of patient visits using CPT & ICD-10 coding standards.
• Counsels and educates patients and family members regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow-up as appropriate for each visit. Verify and document the patients understanding of diagnosis, treatment and follow-up recommendations.
• Perform minor procedures and in-house lab work in accordance with Heritage Health policy and qualifications.
• Practices safety, environmental, and/or infection control methods.
• Participates in QA/QI activities.
• May be required to work with frequent interruptions and respond appropriately to unexpected situations.
• Heritage Health staff have an active role in our Patient Centered Medical Home model of care. This role is designated as part of the Heritage Health PCMH Care Team.
• Regular and predictable attendance is an essential function of this position.
• Performs miscellaneous job-related duties as assigned.
