    Position Summary:

    Work as part of the multidisciplinary team providing physical therapy treatment under the direction of the Vice President of Therapy Services and the supervising Physical Therapist.

    The successful candidate will function as part of the outpatient therapy department in a non-traditional setting.


    Duties and Responsibilities:

    Implement the Physical Therapy Program in accordance with the state and national regulations and the rules that govern the Intergenerational Care Program under the direction of the Physical Therapist.

    Implement treatment as prescribed by the physician, documenting progress as necessary and modify/update goals under the supervision of the Physical Therapist.

    Assist in a training program for interns.
    Attend meetings as requested. Contribute to an atmosphere of a holistic team approach.

    Maintain and care for equipment, linens, and clinic areas, report need of supplies and repairs needed to the director of the department.

    Maintain flow sheets, client notes, attendance records, and written documentation as directed by the Director of Rehabilitation and Physical Therapist.

    Coordinate therapy services with other program schedules.
    Participate in continuing education and professional programs.
    Assist in program areas other than Therapy Clinic as needed.
    Provide for other duties as needed/requested by supervisor.

    Document services in accordance with accepted regulatory and professional standardsQualificationsEducation/Training:
    Current Physical Therapist Assistant license in the State of WisconsinNational CertificationPTA Associate Degree or new graduatesSkills/Abilities:Proficient in physical therapy treatment and documentation
