We are seeking a full-time Pharmacist for our Meds-to-Beds team in Nampa, Idaho. This is a dynamic fast-paced environment with frequent movement between the outpatient pharmacy ED inpatient units and urgent care High levels of direct patient interaction with opportunities to make a meaningful impact on medication understanding and adherence A blend of clinical decision-making operational coordination and patient education Close collaboration with nurses physicians case managers and pharmacy technicians to support smooth and timely discharges The ability to reduce readmissions improve patient satisfaction and enhance continuity of care through proactive medication management A role that requires strong communication skills adaptability and a patient-focused mindsetJob description
