Physical TherapistPRN - As Needed AbleHearts Rehabilitation services are coming this month to Waterford Estates We are building a team that will help residents live the life they want and to help them maintain their independence. Be part of the AbleHearts Rehabilitation multidisciplinary team that will be committed to offering a comprehensive approach to our residents' care, addressing every aspect of their daily living. Our community has an opportunity for a driven, passionate, and caring Physical Therapist.

The responsibilities of the Physical Therapist include:

• Accurately assessing the needs of all patients and carrying out an individual progressive treatment program
• Recording all pertinent information regarding the treatment program
• Attending and contributing to patient staffing and utilization reviews to coordinate a total patient plan of care.

Requirements:

• Must have a Doctorate degree in Physical Therapy from an accredited college or university. Registered by the American Physical Therapy Association, Inc.
• Must hold and maintain license to practice in our state
• One full year of experience in an assisted living facility or similar setting is desired
• Ability to relate to and motivate patients for productivity
• Able to cooperate with patients, families, peers, healthcare professionals, and other staff
• Flexible in adaptation to patient or environmental change
• We are a vaccinated care facility. New hires will need to provide proof of vaccination or other acceptable documents prior to starting employment
