- Provide exceptional patient care, ensuring a high standard of medical service
- Perform health assessments and screen for preventative health measures
- Actively manage common and complex conditions, adapting treatment plans as necessary
- Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and healthcare needs
- Formulate and document care plans based on thorough assessments and medical knowledge
- Explain medical procedures and plans of care to patients and their families, ensuring they understand the treatment process
- Prescribe medication or other forms of treatment according to patient needs and medical guidelines
- Participate in an on-call rotation as needed to provide continuous patient care
- Communicate assessment findings and collaborate with the healthcare team to identify gaps in care and improve patient outcomes
- Board eligible or board certification in Internal Medicine, Family Medicine, or Geriatrics
- Current CT
- DEA license (or eligibility for registration)
- Demonstrated interest and passion for value-based care delivery models
- Strong oral and written communication skills to effectively interact with patients, families, and the healthcare team

As a Primary CarePhysician, you will be responsible for managing patient care and treatment incollaboration with other team members.
