    Description

    Job Title

    A registered nurse plays a vital role in providing high-quality patient care. As part of our team, you will be responsible for assessing patients' physical and psychosocial conditions, developing individualized plans of care, and implementing these plans in collaboration with the healthcare team.

    Responsibilities:

    • Assess patients' physical and psychosocial conditions on an ongoing basis throughout the continuum of care.
    • Develop, communicate, and document individualized plans of care reflecting patient problems and interventions to achieve desired outcomes.
    • Implement and coordinate these plans of care in collaboration with the healthcare team.
    • Evaluate the effectiveness of plans of care, patients' responses to care, and readiness for discharge.
    • Plan for education by assessing individual learning needs and readiness, providing educational opportunities, and documenting outcomes.

    Requirements:

    • Graduate of an accredited school of nursing; BSN preferred.
    • Valid State of Connecticut Registered Nurse license.
    • BLS certification required. ACLS certification required within 6 months.

    Estimated Salary:

    $63,000 - $85,000 per year.
