    Job Title: Licensed Practical Nurse/Licensed Vocational Nurse

    About the Role:

    We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team at ResCare Community Living. As an LPN/LVN, you will play a vital role in providing high-quality patient care to individuals with intellectual, developmental, or cognitive disabilities.

    Key Responsibilities:

    • Provide 1:1 care for clients, following their individualized plan of care while maintaining their dignity and independence.
    • Monitor clients' vital statistics and report any abnormal findings to the case manager and physician.
    • Administer medication, feedings, oxygen, and ostomy care as needed, per client's plan of care, and document results.
    • Complete routines established by other care professionals, such as Physical Therapist, Occupational Therapist, and Speech Therapist.
    • Evaluate and report possible communicable diseases or infections, ensuring infection control policies are maintained.
    • Accurately document nursing actions and communicate with family, case manager, and physicians regarding progress and outcomes.
    • Order supplies and medications as needed to ensure adequate amounts are on hand.
    • Ensure all patient needs are met according to physician instructions.
    • Monitor incidents involving individuals, complete incident reports timely and accurately, and inform case managers.

    Requirements:

    • Completed post-secondary coursework to facilitate licensure as a LPN/LVN.
    • Knowledge of individual service plans, including understanding of client rights and health needs.
    • Proof of current license as a LPN/LVN in good standing from the state(s) of practice.
    • CPR certification.
    • Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
    • In-depth understanding of patient care policies and regulations.
