    Description
    Connected Home Health & Hospice, a Medicare-certified 5-Star Agency, is seeking an experienced and caring Home Health Physical Therapist (PT). We are looking for someone just as passionate as we are about providing life-changing service.

    • No regular After-Hours/On-Call responsibilities. Mon-Fri schedule + weekends and Holidays off.
    • Independent scheduling allows for flexibility managing your own schedule.
    The Home Health PT provides one-on-one physical therapy services to patients in their home. Part of Home Health PT essential duties include development and implementation of Plan of Care for positive patient outcomes; patient and family/caregiver support and education' cohesive and collaborative care with the Nursing and Therapy Team. Part-full time, $46+ DOE
    • Physical Therapist (PT) licensure in Oregon
    • 2+ Years PT experience is preferred
    • Ability to work independently and demonstrate critical thinking
    • Current BLS certification from AHA
    • Be a licensed driver with an insured automobile in good working order
