    Metro is seeking a motivated and passionate Per Diem Physical Therapist to provide treatment in Home Care. If you are a Physical Therapist looking to advance your career, we encourage you to apply.

    Requirements

    • Home health experience is preferred but not required - We will provide you with a mentor to assist with documentation and compliance.
    • Accept patients within your designated geographical area of coverage.
    • Develop and implement individually designed programs to restore overall function and independence in the home setting.
    • Collaborate with physicians, families, and patients to establish a plan of care/goals.
    • Timely and accurate documentation of visits.
    • Effectively communicate discharge plans with family members and other team members.

    Qualifications

    • Graduate of a Physical Therapist program.
    • NY Physical Therapist License or eligibility.
    • Evaluate patients' need for physical therapy and develop an appropriate treatment plan.
    • Provide care and treatment to patients based on physician orders, agency policies, and professional standards of care.
    • Instruct family members, PTAs, Home Health Aides, and other caregivers in appropriate physical therapy techniques.
    • CPR/First Aid certifications required.
