    Physical Therapist - House Calls
    Get your career - and your patients - moving. As a Physical Therapist for our House Calls/Home Therapy program, you would provide direct outpatient care in the home for a brief period prior to transitioning the patient into a clinic post-surgical intervention. In this role you would evaluate case history, functional capabilities, limitations, and restrictions and complete documentation in accordance with case management model to develop and deliver an effective rehabilitation plan in collaboration with the referring and/or attending physician. This role may also treat in physical clinics as needed. This is a full-time position.

    • Diploma from a CAPTE-approved Physical Therapy Program
    • Physical Therapy state license (or in process); new grads welcome
    • Dedication to providing exceptional quality of care to each of your patients
    • Ability and desire to treat patients in their home
    • Post-operative care specialization
    • No OASIS needed What You'll Do:
    • Perform comprehensive, caring evaluations on new patients, establish achievable goals and develop actionable treatment plans
    • Provide on-going physical therapy services that effectively and efficiently comply with treatment plans
    • Take ownership of obtaining necessary additional information and suggesting revisions in treatment as appropriate
    • Document all evaluations, treatments and other pertinent patient interactions
    • Skillfully and safely progress patients through their levels of treatment until successful completion of care
    • Educate and counsel patients of their evaluation and treatment plans
    • Engage with patients for a more impactful, effective, sustainable treatment experience
