    Summary



    The VA Northeast Ohio Health Care System is recruiting for Nursing Assistants. The Nursing Assistants will function within the Housing and Urban Development-Veterans Affairs Supportive Housing Program (HUD-VASH) in Social Work Service. This is an intensive case management program for homeless veterans, aimed at providing psychosocial services to veterans while residing in stable housing through HUD's Housing Choice Voucher Program.



    Duties


    The Nursing Assistant duties include, but are not limited to:

    • Functions as a member of the HUD VASH team and assists licensed nursing staff and others in the care of the patient receiving HUD VASH Services. Patient assignments involve more complex nursing needs, which could vary within a range of predictable requirements.
    • Cares for Veterans in accordance with established nursing policy and procedures, standards of care, principles of cultural transformation, and the plan for care with self direction and minimal supervision.
    • Enters specific information into CPRS as needed. Enters information regarding care rendered, condition and progress of the patient.
    • Performs direct supportive care to patients, and or basic education in individual and group settings. Some examples include education on housekeeping, ordering medications and personal care.
    • Observes patients physical and or emotional status and report deviations to the nursing or supervisory designee in a timely manner.
    • Performs a wide variety of duties aimed at increasing comfort, and ensuring the safety/well-being of the patient.
    • Assists with well checks of the Veterans with HUD VASH staff.
    • Assists with monthly inspections, and or other facility/program needs to maintain housing and independence in the home.
    • Assists with the education of specific health teaching, such as reminding diabetic patients on the importance of skin care, follow-up appointments, and reinforcement of exercise and care.
    • Supports Veterans in self-care activities within their individual capacity, and treatment plan goals.
    • Maintains a clean and safe environment of care.
    • Escorts Veterans to and from patient appointments, and in daily activities.
    • Performs administrative duties and utilizes all resources to ensure charting is completed in a timely manner.
    Work Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m.
