- Collaborate with physicians and supervise patient care provided by LPNs, CNAs, and aides
- Conduct patient assessments, monitor progress, and communicate changes to the physician
- Educate patients on their care plans, medications, therapies, diet, and exercise regimens
- Train family members on the patient's care plan, medications, and home safety measures
- Facilitate goal achievement and coordinate discharge processes
- Active RN license in Colorado
- Graduate from an accredited nursing program
- Minimum of 1200 hours of RN experience
- Healthcare CPR/BLS certification (with a demonstration course)
- Preferably, experience in home health care
- Strong organizational skills
- Capability to manage patients with chronic conditions
- Successful completion of background check and drug test (including THC screening)
- No COVID vaccination mandate
