    Registered Nurse (RN) Case Manager

    Opportunities in: Fort Collins, Colorado

    Compensation: $30-$150 per visit (varies by visit type), PLUS mileage and travel compensation with on-call requirement

    As a Home Health Registered Nurse, here's an overview of your responsibilities:

    • Deliver in-home care to patients post-hospitalization or discharge from healthcare facilities
    • Collaborate with physicians and supervise patient care provided by LPNs, CNAs, and aides
    • Conduct patient assessments, monitor progress, and communicate changes to the physician
    • Educate patients on their care plans, medications, therapies, diet, and exercise regimens
    • Train family members on the patient's care plan, medications, and home safety measures
    • Facilitate goal achievement and coordinate discharge processes

    Essential qualifications for Home Health Registered Nurses:

    • Active RN license in Colorado
    • Graduate from an accredited nursing program
    • Minimum of 1200 hours of RN experience
    • Healthcare CPR/BLS certification (with a demonstration course)
    • Preferably, experience in home health care
    • Strong organizational skills
    • Capability to manage patients with chronic conditions
    • Successful completion of background check and drug test (including THC screening)
    • No COVID vaccination mandate
