Summary:
Presbyterian Is hiring an RN Care Manager for the Heart Group Clinic.
The Care manager provides clinical nursing expertise in areas of care practice, consultation, interdisciplinary collaboration and specialty program development. Works with interdisciplinary clinical team and leadership to facilitate quality patient care of patients across the continuum.
