Shift: 6:30 AM - 5:00 PM

Experience:

• 24 months of role experience is required with some in the last 12 months.

• Must have at least 22 months of Cardiac Cath Lab experience.

• Experience with Epic is preferred from any number of months.

Requirements:

• Candidates must have a California license (required for submission).

• This role may require floating to additional units and locations

• Local & travel allowed. Candidates living <50 miles from facility will be processed as local.

• Flu vaccination required after submission. Religious and medical declinations accepted.

• 3 months gap required between for Travel at Facility: Providence Saint Joseph Hospital Eureka - Dolbeer Street.

• 6 months gap required between for Staff at Program: Aya Program - Providence Health & Services and no current placement allowed at Program: Aya Program - Providence Health & Services.

Special Requests: RTO must be approved at time of submission. Holidays: traveler must be willing to work 2 out of the 3 major holidays: Thanksgiving, Christmas, and New Year.

Shift & Scheduling: Weekend REQ: NO. Call REQ: Yes. Up to 2 weekends in a 6 week schedule. Uncontrolled On-Call (Off-Site): $15.00. Controlled On-Call (On-Site): regular bill rate. Provider must arrive within 60 minutes of receiving call. Preceptor Rate: $1.00 in addition to bill rate.

Floating Requirements: Candidates will float within hospital or to an affiliate hospitals near by. Travelers may be expected to float between sister facilities nearby your contracted facility. Travelers' refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours.
