- Case Management services for injured, ill, or recovering clients.
- Provide clinical case direction, establishing specific rehabilitation objectives and goals towards physical and psychological recovery.
- Monitor and evaluate rehabilitative progress and obtainment of goals.
- Collect and record utilization metrics accurately and provide monthly reporting.
- Assess requirements for medical equipment, therapy, and rehabilitative services for clients.
- Utilize electronic medical records and telemedicine technologies.
- Communicate effectively with the team, providers, and clients.
- Perform ad hoc duties as directed by the Program Manager.
- Formulate timely case reports presenting factual information and analysis.
- TS/SCI with polygraph clearance and US Citizenship.
- Bachelor's degree in Nursing or a Medically Related Field with 5 years of case management experience.
- Registered Nurse, Board-Certified Nurse Practitioner, or Board-Certified Physician Assistant.
- Experience in care delivery, psychosocial support, and reimbursement systems.
- Understanding of rehabilitative strategies and ability to solve complex medical issues.
- Effective communication and relationship-building skills.
- Experience in writing clear reports and effective time management.
- Decisive decision-making skills.
- Preferred to have a Certified Case Manager (CCM) certification.
