- Provide patient care such as bathing, nail/skin care, and dressing/undressing
- Ensure positive, respectful, and confidential communication with patients and their families
- Take and record vital signs, and assist with medication administration
- Plan and prepare meals, shop for necessities, and assist with feeding when necessary
- Assess and report any changes in the patient's condition to the RN Case Manager
- Assist with mobility, exercise, transfers, and other therapy activities
