• Provides comprehensive case management and care coordination for a diverse panel of patients with identified needs spanning age, condition, and population variables
• "Coaches" (rather than "teaches") patients to improve their health behavior to attain health-related goals, while working as part of a health care team (Providers, Nurses, Social Workers, Patients, Family, Caregivers, etc.)
• You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
• Graduate from an accredited school of nursing
• Active, unrestricted licensure as a Registered Nurse in a state
• Current state's driver's license with the ability to travel to hospitals, skilled nursing facilities, patient's homes, and other sites where patients receive care as needed
• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.

Preferred Qualifications:

• Certification in Geriatric Nursing or Geriatric Case Management
• Current state's driver's license with the ability to travel to hospitals, skilled nursing facilities, patient's homes, and other sites where patients receive care as needed
• 2+ years of nursing experience, preferably with older patients
• Proficient in computer use, the Internet and health information technology
• Demonstrated excellent communication, interpersonal and organizational skills, with a flexible and creative approach to problem solving
• Proven ability to work effectively both independently as well as a member of an interdisciplinary team

QUALIFICATIONS

• BLS and ACLS
• National Institutes of Health (NIH)
• Certified Principal Investigator (CPI) Certification
• Sound knowledge of anatomy, physiology, pathology, and medical terminology.
• The ability to stand for extended periods.
• Strong technical skills.
• Knowledge of medical terminology
• Passionate about patient care
• Requires a minimum of two years of current RN Case Management experience.
• Graduate from a fully accredited RN program
• Active unencumbered RN California state license
• One year of Epic experience
