    The Terraces of Phoenix, a HumanGood life plan community named "Best Continuing Care Retirement Community 2024" by U.S. News & World Report is hiring a Nurse Supervisor LPN in Assisted Living.
    Monday
    • Friday, 8:00 AM to 4:30 PM This position requires being on call ** $61,500 $92,500/year (DOE)The Nurse Supervisor LPN/LVN is responsible for the nursing needs of residents, while actively incorporating our "Philosophy for Person-Directed Care.
    " Additional essential functions of this position include, but are not limited to:

    Facilitates nursing care provided to all residents in our Assisted Living Department and includes residents in decision making processesLeads the Caregiver and LVN team and ensure that all procedures are followed during emergency situationsActively engages residents in participation in daily activities which are part of resident's rhythm of daily life (including desired hygiene, grooming, and mobility)Assesses resident's needs at the time of admission and continually evaluates residents' condition during the duration of their stay and promotes the highest functional level of independence possible (keeping in compliance with state and federal regulations, and the community's safety proceduresCoordinates the residents' plan of care with the interdisciplinary team (IDT), physicians, medical personnel, and resident's family/responsible partyInteracts with other departments such as laboratory, radiology, dietary, facilities, information technology, pharmacy, social services, activities, etc.

    Makes necessary documentation corrections as a result of medical record auditsAssists in providing Resident and family educationTo be successful in the role, you would have:

    Current LPN/LVN license in the state for which applyingCurrent CPR certificationCurrent/prior Supervisor experience in Assisted LivingCurrent/prior nursing experience in senior care, assisted living, or skilled nursing preferredWhat's in it for you?As one of the largest nonprofit owner/operator of senior living communities in the country, we are more than just a place to work.
