    The Per Diem Job Coach will provide supervision, support and training to people with differing abilities served by SARAH Inc in the areas of New Haven, Orange, Milford, Guilford and Branford. Develop and implement employment activities that are person centered and promote positive outcomes. Provide the environment and supports necessary to promote appropriate social and vocational skills as well as positive behaviors that will enhance skills, enrich lives, and foster community presence & participation.

    HOURS OF DUTY: Monday-Friday 7am-3pm, Per Diem

    ESSENTIAL FUNCTIONS:

    • Ensure the health, safety and well-being of people served by SARAH Inc. in accordance with the individual service plan and in compliance with agency policies and procedures.
    • Provide transportation for assigned service participants in accordance with SARAH Inc. safe-driving policies and practices.
    • Facilitates vocational learning through assessments, on-line curriculum and volunteer and or assessment sites.
    • Interface/interacts with employers, volunteer and/or assessment site contacts.
    • Represent the agency in a professional and positive light at all community partners.
    • Develop and implement employment activities that are person-centered and promote positive outcomes.
    • Maintain medical, social and programmatic files by recording each person's progress and documenting other required information. This includes, but is not limited to: service goals, attendance records, behavioral data, progress notes, quarterly and annual reports, and other documentation as required or needed.
    • Develop and maintain positive relationships with service participants, families, co-workers and other community representatives.
    • Other duties, assignments and work locations as assigned.
    Requirements

    QUALIFICATIONS:
    • High school diploma or equivalent required; Bachelor degree in related field preferred;
    • Knowledge of learning and behavioral processes helpful.
    • Must be over 18 and possess a valid Connecticut driver's license, have an acceptable driving history and be able to utilize your own vehicle for SARAH business as defined by SARAH Inc.'s policy.
    KNOWLEDGE, SKILLS AND ABILITIES:
    • Knowledge of person-centered, outcome-based services for people with differing abilities
    • Knowledge of company policies and procedures and applicable regulatory standards
    • Knowledge of community resources
    • Skill in working with people with differing abilities
    • Skill in operating standard office equipment, computer and various software packages.
    • Skill in implementing programs & activities as outlined in the individual support plan
    • Skill in communicating effectively in a courteous and professional manner
    • Skill in writing clearly and concisely to maintain accurate records
    • Ability to attend to the physical and personal needs of adults with differing abilities
    • Ability to demonstrate good communication and interpersonal skills necessary to provide guidance and instruction to adults with significant challenges;
    • Ability to work successfully, both independently and in a team environment
    • Ability to problem-solve effectively
    • Ability to maintain confidentiality
    • Ability to use adaptive and other equipment
    • Ability to maintain a patient and positive attitude
    • Ability to drive and travel to varying locations as needed
    REQUIRED PHYSICAL EFFORT: Must be able to sit, stand and walk at an assigned location(s) and work for extended periods of time; bend/stoop, kneel and balance frequently; climb, reach above shoulder level; push/pull, squat and crouch occasionally; with assistance, lift, carry and position individuals of various weight. Must be able to lift and carry up to 50 pounds occasionally. Must be able to drive multi-passenger vehicles for extended periods throughout the day.
