    Position Purpose: Provides rehabilitation services at the Pediatric and Adolescent Transformative Health (PATH) Clinic. PATH is a 12-member collaborative led by Saint Anthony and will provide an innovative structure for a Behavioral Health Home (BHH) serving children, adolescents, and families on the west and southwest sides of Chicago.

    Job Responsibilities:

    • Evaluates and treats patients using the principles and practices of physical therapy in accordance with the patient's age, developmental level and diagnosis.
    • Assess and treat infants, children, and teenagers up until the age of 17 with developmental, behavioral, congenital, neuromuscular, and orthopedic disorders.
    • Use special instruments and qualitative and quantitative assessment methods to analyze and diagnose the nature and extent of the impairment.
    • Evaluate and assist in the design of adapted equipment for each assigned child along with other members of the Rehab team.
    • Perform in-depth evaluations, develop treatment plan, and reassess each patient's progress and change treatment plan if needed.
    • Maintain records relating to attendance, initial evaluations, progress notes, and discharges.
    • Work with family members to recognize and change behavior patterns that may be impeding progress.
    • Demonstrates sensitivity to cross-cultural issues.
    • Maintains patient confidence by keeping information confidential.
    • Maintains safe and clean working environment by complying with procedures, rules, and regulations.
    • Protects patients and employees by adhering to infection-control policies and protocol.
    • Complies with federal, state, and local legal and certification requirements by studying existing and new legislation.
    Minimum Requirements
    • Active Illinois Physical Therapy License
    • Masters in Physical Therapy Required. Doctorate Preferred
    • Clean DOJ/FBI background check
    • CPR Certification
    • At least 1 year or experience working exclusively with a pediatric population.
    • Certification from the American Board of Physical Therapy Specialties in Pediatrics (or at minimum In Progress towards this certification)
    • Participation in continuing education in pediatric physical therapy subjects
    • Bilingual in English/Spanish a plus
    • Ability to communicate with staff at all organization levels.
    • Full Vaccination Status (COVID-19)
