- Enhancing and maintaining communication between team members and referral sources
- Traveling to referring facilities and completingadmission assessments
- Completing admission assessments remotely for out of area referrals via phone / fax
- Developing and maintaining relationships with referring facilities
- Screening patients for Medical Necessity, IRF 60%, and LTACH 25% rule compliance
- Receiving referrals directly from stakeholders
- Communicating information with admission coordinators
- Coordinating with referring facility staff, physicians, and patients / families
- General marketing and education
- Bachelors degree or Masters degree preferred in a clinical field.
- Three years of clinical practice required. Working knowledge of insurance coverage, medical diagnoses, pharmacologic treatments, and disease progression and recovery required.
- Must be able to work independently and make independent decisions, yet collaborate with team members when needed. Demonstrates judgment and initiative to assess the physical and psychosocial functioning level of patients in the context of family support. Must have excellent communication skills, organizational skills, and time management skills.
