About Us:

AuSable Valley Community Mental Health Authority is a public mental health agency providing quality prevention, education, and mental health services to Iosco, Ogemaw, and Oscoda Counties in northern Michigan.

Job Summary:

The primary goal of this position is to support individuals with severe and persistent mental illness, serious emotional disturbance, and/or intellectual/developmental disabilities to become active and integrated citizens within their communities.

Responsibilities:

• Assist individuals in identifying personal preferences and implementing a plan of support that honors these preferences.
• Collaborate with individuals, families, community members, case holders, and consultants to provide supports and skill building consistent with the Plan of Service.
• Help individuals to increase independent functioning and decrease the need for treatment in more restrictive settings.

Requirements:

• Possession of a driver's license valid in the State of Michigan.
• Vehicle insurance as required by Michigan Law.
• Good driving record.
