- Promote patients' health maintenance and restoration to provide quality, cost-effective outcomes.
- Initiate a discharge plan to meet immediate post hospital needs, as well as visualizing long term needs, to optimize health maintenance and prevent readmission.
- Educate patients, family, and staff.
- Collect, analyze, and interpret data to evaluate current processes, and alter practices that don't achieve optimal patient care and customer service.
- Seek learning opportunities to expand current clinical knowledge.
- Seek learning opportunities to expand cost containment strategies.
- Two (2) years' Med/Surg or related experience
- Clinical case management experience (preferred)
- Bachelor of Science in Nursing (BSN) from an accredited school of nursing
- Current RN license in the State of Vermont
