    Social Service Coordinator

    Department: Social Services FLSA/Classification: Exempt/SalaryReports to: Health Care Center AdministratorDirect Reports: N/A

    Summary:

    Responsible for planning, organizing, implementing, evaluating, and directing the overall operation of the Social Service Department in accordance with current federal, state, and local standards, guidelines, and regulations, and the established policies and procedures. The Social Service Coordinator also educates residents and families on available services, develops contact with service providers and agencies for resident referrals, and monitors the provision of services.

    Essential Duties and Responsibilities include, but are not limited to:

    Operations:

    • Assess the social service needs of residents, determine goals, and provide services to meet identified needs.
    • Counsel residents and family members as required to cope with changing physical/medical conditions, care level changes, death, etc.
    • Schedules and attends care planning meetings to evaluate residents' problems, and needs, and to plan solutions.
    • Advise on individual activities based on social, emotional, and spiritual needs of residents, and meeting special needs created by various conditions or impairment.
    • Orient new Wesley Willows residents and their families with the accompanying adjustments.
    • Develop the social history of each Wesley Willows resident residing in the Health Care Center and document in charts social service needs and progress notes.

    Administrative:

    • Maintain a minimum of forty hours per week, or as many hours as required to meet job expectations, based on a Monday through Friday schedule, including weekends, holidays, and other alternate hours as needed.
    • Ensure compliance with all federal, state, and local laws and regulations.
    • Maintain a safe and productive work environment.
    • Participate in facility surveys made by authorized government agencies.
    • Assist with the plan of correction for deficiencies noted during survey inspections.

    Key Competencies:

    • Must possess knowledge of regulatory compliance and continuing Care Retirement Community operations
    • Maintains a courteous and professional manner through interactions with others.
    • Uses a resident and customer-focused approach to problem-solving, goal-setting and management philosophy.
    • Uses discretion in handling confidential information, incorporating all federal, state, and local privacy and confidentiality requirements
    • Must be sensitive to the needs of older adults and enjoy working with a senior population.
    • Proficient human relations skills with experience in interviewing/selecting, evaluating, training, coaching, and motivating staff.

    Minimum Education/Skills/Experience/Credentials:

    • Bachelor's degree in social work, the social sciences, or a related field preferred.
    • Prefer 2-5 years of experience in activities or program coordination, counseling, and/or social work.
    • Good interpersonal skills both written and verbal.
    • Ability to interface with residents, family members, and employees professionally.
    • Computer literacy required.

    Physical Requirements:

    Must be able to sit for up to 3 hours at a time. Must be able to frequently lift 10 pounds and occasionally lift 50 pounds when moving records and other office supplies. The job requires the use of hands for grasping, lifting, and typing.

    Other federally regulated requirements:

    • Must be willing to undergo pre-employment background check, including (MVR) moving vehicle records.
    • Must have or be willing to receive the Flu-Shot Vaccine, and TB testing
    • Must be willing to undergo pre-employment drug screening, to include cannabis.
    • Must be willing to undergo biometric screening.
