    Job Requirements

    American Heart Association
    BLS certification required for all direct patient care personnel
    License/Certificate verification must be dated within 30 days of start
    Fit Test required
    Trinity 10 Panel Drug Screen required within 30 days of start date
    Trinity Code of Conduct Form must be signed
    Standard i9 completed by the agency
    Confidentiality and Information System Security Agreement must be signed
    Integrity and Audit Services Form signed within days prior to start
    Non Employee Upload Form must be completed
    Specialty Specific Competency Testing must be completed within a year
    Color Vision test required
    COVID-19 Vaccine proof or declination required
    MMR vaccination proof or IgG titer required
    Hepatitis B vaccination series required prior to start
    Influenza Vaccine proof or declination required
    2 Step PPD OR IGRA (Quantiferon/Tspot) required
    Varicella vaccination proof or IgG titer required
    Physical/Health Statement from exam within last year required
    Resume, Skills Checklist within a year, 2 Professional References required
    Conflict Of Interest Disclosure Form must be signed
    Criminal Background Check within last 7 years required, dated within 30 days of start
    Education Verification required
    EPLS report dated within past 30 days required
    OIG report dated within past 30 days required
    Joint Commission CORE Exam within a year required
    NSO report dated within 30 days of start required
    System Access Form must be completed and uploaded one week before start
    Skills Checklist provided by agency for clinical contractors
    Tdap vaccine or declination required
    Child and Adult Dependent Abuse Training required for Iowa
    Iowa Sing Report required within 30 days prior to start
    OFAC check must be completed within 30 days of start
