Job summary

We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have experience in assisted living, nursing homes, and hospital settings, with a strong focus on patient care.

Responsibilities

• Conduct assessments to identify clients' needs and develop appropriate care plans.
• Provide counseling and support to individuals dealing with trauma, PTSD, and other mental health issues.
• Collaborate with healthcare teams to ensure holistic patient care in assisted living facilities, nursing homes, and hospitals.
• Implement therapeutic interventions using motivational interviewing techniques.
• Advocate for clients' rights and access to necessary services and resources.
• Maintain accurate documentation of client progress and treatment plans within established systems.
• Facilitate group therapy sessions and support groups as needed.
• Engage with families to provide education on coping strategies and resources available for their loved ones.

Qualifications

• Bachelor's or Master's degree in Social Work or a related field.
• Experience in assisted living, nursing home, or hospital settings preferred.
• Knowledge of PTSD care practices and applied behavior analysis techniques is a plus.
• Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
• Excellent communication skills, both verbal and written.
• Ability to work collaboratively within a multidisciplinary team environment.
• Proficiency in utilizing electronic health record systems for documentation purposes.
• A valid social work license may be required depending on state regulations.
