    Summary



    The LPN is expected to exercise technical judgment independently and requires minimal or no guidance and direction. Completed work generally needs little review by a registered nurse for technical soundness, appropriateness and conformity to policy and accepted nursing standards.



    Duties


    The Licensed Practical Nurse (LPN) is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic life care plan. The LPN will initiate appropriate actions based on the treatment plan and his/her observation of the veteran's condition. The LPN assists with providing patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.

    Duties of this position include, but are not limited to:

    • Provide a full range of LPN/LVN care to patients with a variety of physical and psychological needs.
    • Work independently and provides modalities to veterans that have multiple complex medical and or emotional conditions.
    • Demonstrate adherence to VA and Nursing policies/procedures and standards, and maintains competencies consistent with role responsibilities.
    • Assist the RN/HCP and other staff in proactively identifying and addresses the veteran's needs, and is able to readily recognize the more subtle signs & symptoms of changes in the veteran's condition, and takes appropriate action.
    • Participate in the development of educational material for providing veteran, family and caregiver teaching.
    • Teach and document health concepts to veteran, including learning needs and readiness to learn. Is self-directed in seeking out and participating in learning opportunities for the veteran.
    • Perform treatments, procedures and the use and reprocessing of reusable medical equipment appropriate to assigned duties.
